Who We Are:
PSKS was founded in 1995 by a group of homeless youth who wanted their voices to be heard by policy makers who did not understand the impact of their decisions. With this bottom-up approach, PSKS found its niche – empowering youth who are too often excluded from mainstream services. At the heart of the PSKS community is our CORE Membership of participants – an inclusive governance structure giving voice and power to our youth to influence organizational programs and policies. Young people at PSKS develop a sense of ownership and pride being actively engaged to end the challenges of personal and community homelessness.
Capitol Hill Case Management program works with high-risk youth and young adults not currently served by other agencies. The case manager refers to and advocates for participants needing housing, employment, education, legal and medical services and to other relevant community resources. CM also provides valuable one-on-one peer support groups and emotional support for the many youth who might not find it elsewhere.
Capitol Hill and PSKS Shelter Case Management Duties and Responsibilities:
(approximately 20 hours per week)
- Assess Participant needs; advocate for and refer participants to housing programs, legal services, chemical dependency treatment, counseling services, employment programs
- Create Individualized Support Agreements as part of the intake process
- Create, and plan new participant orientations in collaboration with other key staff
- Be present at daytime drop in hours and occasionally on site during late night shelter hours to engage youth for services/conduct intakes and/or set appointments for intakes or other external resources meetings; show up on time; reliable
- Accompany participants to off-site appointments; consistent follow up with referral agencies
- Assist participants to apply for government and other benefits
- Keep accurate, detailed case files daily (audited yearly by the city)
(approximately 8 hours per week)
- Attend outside provider meetings to keep up with current trends and resources
- Attend external community advocacy meetings
- Collaborate with other staff in producing grant proposals and reports
- Participate in weekly staff meetings; training programs; supervisory sessions
- Ensure participants adhere to Safe Spaces Rules and Accountability process
- Adhere to agency policy, procedures and professional code of ethics
- Assist with any other duties deemed necessary by the Executive Director
